Inpatient Case Manager
Siskin Hospital for Physical Rehabilitation is seeking a full-time Inpatient Case Manager to add to the team. Their primary responsibility is to coordinate the provision of care by working closely with both internal and external individuals / organizations. The successful candidate will meet the patient and their caregivers / support system to orient them to the process of rehabilitation and provide an overview of their specific program. They will complete the initial evaluation, share this information with the treatment team and will be instrumental in developing a treatment plan that has the goal of the patient returning home. The Case Manager monitors the patient and family’s progress towards their short and long-term rehab goals, discharge plan and outcomes of care. They will ensure that all DME, medical equipment and follow up services are arranged prior to discharge in a manner that meets the patient and caregiver’s needs. The Case Manager will educate the patient on follow up therapy services available and makes referrals to OP and / or Home Health Services. They also provide information for PAI data collection process for IRF through the initial assessment and discharge summary documentation forms, and acts as the patient’s advocate during the inpatient stay. They ensure the patient’s involvement throughout their rehabilitation program, and make arrangements for transfer to alternative levels of care when discharging to home is not an option. Minimum qualifications or equivalents
Education:
Minimum of an Associate’s Degree in a health care or related field preferred
Experience:
Minimum of 1 years’ experience in the field of Healthcare with experience in a hospital or rehabilitation setting.
Inpatient hospital experience preferred.
