About the Role:
As the Case Management Supervisor, you will:
Lead and Inspire: Supervise and support a team of talented discharge planners, fostering growth and development through effective hiring, training, and evaluation.
Drive Excellence: Ensure compliance with CARF and CIHQ standards through regular chart audits and uphold our high-quality care standards.
Collaborate and Communicate: Engage with interdisciplinary teams, attend leadership meetings, and contribute to strategic hospital initiatives.
Make an Impact: Provide on-the-floor support as needed, ensuring continuity of care and exceptional patient outcomes.
Optimize Patient Journeys: Oversee length of stay (LOS) and skilled nursing facility (SNF) discharges, identifying opportunities for improved care and efficiency.
Educate and Support: Guide families through insurance complexities, offering compassionate and clear communication throughout the discharge planning process.
What You Bring:
Education:
Master’s degree preferred, bachelor’s degree required (Social Work, Human Services, Nursing, or related field).
RN licensure required if applying as a nurse.
Experience:
Proven experience in an acute inpatient rehab setting preferred.
Expertise in clinical EMR systems, staff management, and interdisciplinary team coordination.
Strong background in case management, discharge planning, and resource coordination.
Skills:
Exceptional leadership and organizational skills.
Effective communication—both verbal and written.
A proactive, problem-solving mindset with the ability to multitask in a fast-paced environment.
