Social Services Representative - PRN
Siskin Hospital for Physical Rehabilitation is seeking a PRN Social Services Representative to add to the team at Subacute West. Their primary responsibility is to coordinate the provision of care by working closely with both internal and external individuals / organizations. The successful candidate will meet the resident and their caregivers / support system to orient them to the process of rehabilitation and provide an overview of their specific program. They will complete the initial evaluation, share this information with the treatment team and will be instrumental in developing a treatment plan that has the goal of the patient to have an appropriate and safe discharge plan. The Social Services Representative monitors the resident and family's progress toward short and long-term goals, discharge plan, and outcomes of care. They will ensure that all DME, medical equipment and follow up services are arranged prior to discharge in a manner that meets the patient and caregiver’s needs. Social Services Representative will educate the resident on follow up therapy services available and makes referrals to OP and / or Home Health Services. They also provide information for the MDS data collection process through the initial assessment and discharge summary documentation and acts as the patient’s advocate during the in-resident stay. They ensure the resident’s involvement throughout their rehabilitation program, and make arrangements for transfer to alternative levels of care when discharging to home is not an option. Minimum qualifications or equivalents
Education:
Associate's Degree in a healthcare or related field preferred
Experience:
Minimum of 1 years’ experience in the field of Healthcare with experience in a hospital, long-term care, or rehabilitation setting.
Long-term care or SNF experience preferred.
